Weight management

Use this service to check your eligibility for a referral to an NHS weight management service.

The weight management services support adults living with obesity who also have a diagnosis of diabetes, high blood pressure (hypertension) or both, to manage their weight and improve their health.

You can use this service if you:

• are registered at the surgery

• your first and last name, date of birth, sex, postcode, email and phone number
• if applicable, the details of the person you are completing the form on behalf of
• your height and weight
• your ethnicity
